Please help, I’m about 3/4 of the way to losing my mind. So, originally my o2 (bank one sensor one) was out, replaced it, then a throttle/pedal position code was thrown (it was accidentally cleared, but never came back) but has not reappeared. Now, the P2195 code is being thrown and for the life of me I cannot figure out what the problem is. So far (just looking about and seeing what needed to be replaced) the o2 sensor was replaced, as well as spark plugs, throttle body, valve cover gasket, MAP sensor, and it has a brand new air filter. I have checked for vacuum leaks, but have come up empty handed. Pulled the fuel rail and cleaned injectors after which I cleared the code and it went away for about 40-50 miles then proceeded to return. I’m at a loss and in this economy the $400 I’ve spent could have made a really nice trip to the grocery store lol.

If you can, look at your I/M Monitors thru your DTC connector - using a simple Code scanner - nothing fancy.

Just see if which area of the system still shows NOT READY - if it's Fuel, could be a bad O-seal on an injector - or in Emissions, can be related to a bad Exhaust seal - but that depends on what I/M monitor is calling the P2195 code out.

What we're trying to do here is localize the reason for it using the Monitor to find which system is most affected - then we have a way to know what could be causing the code.

A good clue here is with your injector cleaning - so something with fuel trim or plugged/leaky injector is possible - but if the Exhaust system causes the Code - the injector cleaning only cured a symptom for only a moment.

A lot of this can also translate to Electrical issues in the harness; a bad ground return wire from the Injector-side of the harness to one or more of the injectors or on-to a bad wire in the harness of the O2 sensor - which the OEM one may have been OK, but a bad return will give a bad signal and therefore the PCM scrambles to find answers that can't fit the puzzle-throwing the code.

But knowing the areas it's concerned with; as NOT READY - helps us help you.
